This appointment is most commonly made for new patients with an expanded problem-focused medical health concern and history with a straightforward medical decision-making.
This request requires the interested person to become a new patient of our Doctor. If you cannot visit our office, we are happy to meet with you for this consultation through either teleconference or by telephone.
We will need to initially email to you forms to fill out and ask you to return them completed to us by either regular mail, by fax, or to scan the completed forms into your computer to email as attachments to us. After we receive the completed forms we will confirm your appointment with our Doctor.
